CARDI002776 Capital Cardiology Associates is looking at posting a part-time APP telehealth remote position to help with TCM visits and triage health calls/tasks. Ideally this would be 8:00am-2:00pm or 9:00am-3:00pm Monday through Friday. Position Overview: We are seeking a highly motivated and clinically skilled Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our cardiology team in a remote, part-time telehealth role. This role is primarily responsible for transitional care management (TCM) and longitudinal remote task management, including working the Health Problem task list, with additional support for urgent symptom triage.  The APP will work collaboratively with triage nurses and cardiologists while maintaining a high degree of autonomy in clinical decision-making. This is an excellent opportunity for both experienced providers and new graduates with a strong background in cardiology, emergency medicine, or critical care. Key Responsibilities include but not limited to:

  • Perform Transitional Care Management (TCM) visits (7- and 14-day as appropriate) and ensure completion of all TCM requirements, including medication reconciliation, review of hospitalization records, coordination of follow-up care and testing. Track and manage post-discharge hospital lists to ensure no gaps in care.
  • Respond promptly to urgent patient calls, messages, and clinical tasks related to cardiovascular concerns
  • Actively manage and resolve EHR-based Health Problem tasks, including abnormal results, medication issues, symptom-based patient messages
  • Conduct same-day or urgent telehealth visits for acute symptoms as needed
  • Adjust and manage cardiac medications remotely, ensuring patient safety and appropriate follow-up
  • Coordinate and schedule additional follow-up visits based on clinical assessment
  • Collaborate closely with triage nurses to streamline patient care and escalate to the supervising or primary cardiologist when necessary
  • Communicate and coordinate care with the primary cardiologist as needed
  • Provide patient education on symptom management, medications, and when to seek higher levels of care
  • Maintain accurate and timely documentation in the electronic medical record
Qualifications:
  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA)
  • Active and unrestricted state license
  • Preferred: Minimum of 2 years of experience in cardiology, emergency medicine, or critical care
  • New graduates with strong clinical training and interest in cardiology are encouraged to apply
  • Strong clinical judgment and ability to work independently in a remote setting
  • Excellent communication and patient engagement skills
  • Experience with telehealth platforms and electronic medical records preferred

Compensation:
  • Salary $67,000-$71,500 annually based on experience and qualifications.
