This role focuses on ensuring member compliance with a new STARS measure and confirming successful emergency department discharge. Additional responsibilities may include closing other HEDIS measures, all while delivering compassionate, high-quality virtual care.</p><p></p><p>Additionally, this role may at times work alongside the Pharmacy Operations team to support prescription fulfillment.</p><p></p><p><b>Key Responsibilities:</b></p><ul><li><p><b>Telehealth Patient Care</b>:<span> Provide remote patient care and support through telehealth platforms.</span></p></li><li><p><b>Virtual Assessments</b>:<span> Conduct virtual patient assessments, including reviewing medical histories and current symptoms.</span></p></li><li><p><b>Medical Documentation</b>:<span> Accurately assess and document patient conditions, histories, and care interactions.</span></p></li><li><p><b>Patient Education</b>:<span> Educate patients and their families on medical conditions, medication adherence, and self-care practices.</span></p></li><li><p><b>Care Plan Collaboration</b>:<span> Work closely with other healthcare providers to develop and implement individualized care plans.</span></p></li><li><p><b>Regulatory Compliance</b>:<span> Maintain accurate records and ensure adherence to healthcare regulations and privacy laws; maintain patient confidentiality with medication profiles.</span></p></li><li><p><b>Continuous Education</b>:<span> Stay up to date with medical advancements, telehealth protocols, and best practices through continuing education.</span></p></li><li><p><b>Compassionate Communication</b>:<span> Demonstrate empathy, professionalism, and effective communication in all patient interactions.</span></p></li><li><p><b>Pharmacy Support</b>: <span>Assist in the supervision of remote pharmacy technicians as needed; coordinate the provision of pharmaceutical services and delivery of medications and products to patients; ensure the security of legend drugs; verify the accuracy and appropriateness of prescriptions.</span></p></li></ul><p></p><p><b>Required Experience & Qualifications:</b></p><ul><li><p>Active, clear, and unrestricted Pharmacist license in the State of Florida.</p></li><li><p>Registered Pharmacist in good standing with the Florida Board of Pharmacy.</p></li><li><p>Bachelor’s degree in Pharmacy or Doctor of Pharmacy (PharmD).</p></li><li><p>Minimum of 1 year of experience as a Registered Pharmacist.</p></li><li><p>Strong organizational and leadership skills to support clinical/pharmacy operations.</p></li><li><p>Strong computer skills and familiarity with electronic medical record (EMR) systems.</p></li><li><p><span>Ability to communicate clearly and effectively in both verbal and written forms.</span></p></li><li><p><span>Demonstrated ability to follow standard operating procedures and adapt in a fast-paced environment.</span></p></li><li><p><span>Strong customer service and interpersonal skills, especially in high-volume or high-stress scenarios.</span></p></li><li><p><span>Ability to work both independently and as part of a collaborative care team.</span></p></li></ul><h1><br><br><b>Additional Information</b><br> </h1><p><span>Physical requirements for this role include the ability to work at a computer terminal with monitor, keyboard and mouse for extended periods of time, periodically move away from the computer terminal for in-person meetings, stoop, bend, and reach for equipment and supplies, make frequent repetitive motions required to operate a computer that include the wrists, hands and fingers, and lift, carry, push, pull, and move objects up to 50 pounds.
