As a facilitator, or case manager, you will consult with families throughout the state of Minnesota both in person and remotely. Consultation Services is a service for Community First Service and Supports (CFSS). This is a new program being developed in Minnesota. Consultation Facilitators work as case managers and work alongside the individual and their team to ensure that they have seamless access to services. You will help individuals develop a service plan for themselves to use CFSS services, teach about the program through orientation, provide guidance, conduct meetings, troubleshoot issues, and communicate with individuals and their families. We are looking for great team members who are passionate about person-centered planning, self-direction, and independence for people with intellectual and developmental disabilities. </span><br><br><b><span style="font-family:Calibri, sans-serif;">Schedule:</span></b><span style="font-family:Calibri, sans-serif;"> Monday – Friday, 8:00 am – 4:30 pm.
